POSITION: Life Skills Coordinator/Case Manager, Young Emancipated Adults (YEA) Program
SUPERVISOR: Railton Place Program Director
STATUS: Non-Exempt, Full time, 40 hours/week

OVERVIEW: The Life Skills Coordinator/Case Manager is responsible for intake screening, case management and discharge processing in the Young Emancipated Adults Transitional Housing program. The Life Skills Coordinator/Case Manager will help the participants, aged 21 – 25 years, to achieve stability, increase their levels of income, and help to motivate greater self determination. This includes individual assessments to determine interests and needs and to develop an on-going case plan. The Life Skills Coordinator/Case Manager will help link the participant to outside educational and/or vocational resources in order to reach the goals stated in their individual case plans. The Life Skills Coordinator/Case Manager will work in collaboration with the Program Director, Case Managers, and Mental Health/Substance Abuse Counselor to help the client achieve their goal of independent living.

QUALIFICATIONS:
• Must be at least 21 years of age.
• Bachelor’s Degree in Social work or related field AND one year case management experience. Experience working with emancipated youths a plus.
• Knowledge and experience providing mental health and substance abuse counseling preferred.
• Spanish speakers strongly encouraged to apply.
• Command of the English language and excellent written and verbal skills.
• Ability to maintain confidential and accurate written records of all interactions regarding clients and maintain client files.
• Ability to maintain highest level of integrity and confidentiality in all areas of personnel, programs and operations.
• Be familiar with mental health issues specific to this population.
• Experience working with youth who are in crisis, homeless, economically disadvantaged, mentally ill, and chemically dependent is a plus.
• The ability to write reports and compile statistical data.
• Excellent computer skills with knowledge of MS Word and Excel.
• The ability to cooperate and work in a team with other staff members.
• Energetic, enthusiastic, creative, initiative a must.
• The ability to maintain good rapport, communicate and work effectively with program participants, other staff members, Kroc personnel, and the public from various ethnic and cultural backgrounds. (Works well in multidisciplinary settings.)
• Ability to work independently with minimal supervision.
• Organization, detail-oriented, and neatness is a must.
• Must have a valid California Driver License and insurable to drive Salvation Army vehicles.
• Knowledge of Salvation Army programs, practices, policies and organization desirable.
