DEPARTMENT: Homeless Outreach Programs

POSITION TITLE: Case Manager

SUPERVISOR: Lahaina Officer in Charge, Maui County Coordinator

FLSA STATUS: Non-Exempt, Full Time

JOB SUMMARY:

This position is publicly funded by grants from the County and the State governments. The primary goal of this position is to help clients successfully transition to permanent housing. As Case Manager you will assist with the care and wellbeing of homeless clients by providing basic supplies, services, referrals and case management.

ESSENTIAL QUALIFICATIONS

· Valid driver’s license

· CPR training

· Must have good team management skills

ESSENTIAL FUNCTIONS:

· Work in close coordination with the Lead Case Manager, Lahaina Officer in Charge, Family Life Center staff and Maui County Coordinator as the “Homeless Programs management team” to develop, implement and maintain goal-oriented outreach plans.

· Assist in the management and supervision of all aspects of the Homeless Programs.

· Assess needs for services, encourages use of services, makes referrals, and act as an advocate to enable clients to access needed services.

· Develops trusting relationships with persons who are homeless and acts as their advocate when appropriate.

· Maintain good case notes and work with clients to help them achieve stated goals.

· Collect, compile and maintain necessary data for TSA statistics and State and County grant requirements.

· Input and/or compile all required data for the Salvation Army Databases and the State of Hawaii HMIS data base.

· Maintain client files in accordance with the State and County grant requirements and maintain client confidentiality at all times.

· Attend all available training sessions, county CES meetings, weekly/monthly Homeless Outreach and program staff meetings and attend weekly client evaluations with case managers.

· Develops a good working knowledge of referral agencies eligibility requirements, procedures, waiting periods, and any other information needed in the assistance of clients.

· Foster and maintain good collaboration and cooperation with other service providers.

The position requires sitting; climbing and/or balancing; stooping, kneeling, bending, stretching, crouching and/or crawling; standing; walking; manual dexterity and eye-hand coordination; use of vision; driving a vehicle; pushing/pulling/lifting/carrying 25-50 pounds and occasionally more than 50 pounds; traveling by airplane.

COMMUNICATION DEMANDS:

The position requires talking to co-workers, clients, vendors; requires written communication to and from co-workers, clients, vendor; talking on the telephones; requires responding to written or verbal requests of co-workers, clients, and vendors; requires training/giving verbal and written instructions; requires receiving verbal and written instructions; requires writing/composing written language; requires reading; requires visiting/working at other worksites; requires communication via the latest technologies

SKILLS/TECHNICAL KNOWLEDGE:

Valid Hawaii driver’s license.

Travel off-island as necessary (if needed)

EQUIPMENT NEEDS:

Use office equipment such as computers, photocopiers, scanners, calculators, etc.

DESCRIPTION OF WORK ENVIRONMENT:

Work will be outdoors and indoors at the Lahaina Corps facility or around the island of Maui. Attendance at meetings and trainings, both in Hawaii and on the mainland, may be required.
